Our automated service will call you two days in advance before your appointment to remind you of your appointment and to confirm that you will be able to make the appointment at the scheduled time. It is vital that you confirm your appointment by noon the day prior to your original appointment day. You may call to confirm your appointment by dialing 469-916-0677. You may also use this number to inquire about your appointment or to reschedule if necessary. Appointments that are not confirmed 24 hours prior to the appointment day may be cancelled in order to allow access to that time-slot by another patient. Unfortunately, an unused appointment time-slot is time that is lost and cannot be recovered. Therefore, unconfirmed appointment time-slots may be offered to patients that have an urgent need to see a healthcare physician. In addition, if you do not keep your appointment, and you do not call to reschedule prior to the day of your appointment, you may be charged a missed appointment fee.

There will be a $25.00 charge for all established patient that cancel, or reschedule with less than a 24 hour notice. There will be a $50.00 charge for all new patients that cancel, or reschedule with less than a 24 hour notice. Please bring your insurance cards with you on every visit to the office. If you do not have your insurance card, you will have to reschedule your appointment. If you are running more than 15 minutes late you will also need to reschedule your appointment.

In order to conduct a thorough examination on your first visit, we will need copies of your recent medical records from your other physician(s). This includes labs, x-rays, MRI's, and/or other information that you feel is important to your care. Please contact your healthcare provider(s) to obtain copies in advance to your appointment. You may bring these with you on your appointment day, or have them faxed to 214-716-5283 to the attention of Medical Records.
